Why this happened, in plain terms

We spent several pages criticising other publications for quoting prices without their term, dose and formulation. Then we republished their figures without checking any of the three.

Our evidence model has a status called reported, which means the provider reported it. We applied it to figures no provider had reported to anyone — they were things other websites had written. That is not a labelling nicety. It is the difference between a price and a rumour.

A new status now exists, third-party claim, and figures carrying it cannot be ranked, summed, averaged or compared anywhere on this site. The Embody case shows why: a single number, republished across at least five publications, that does not survive one search.

If you want a figure for any provider below, the honest answer is that you should read it on their own pricing page, on the plan you would sign, at the dose you expect to reach. That is what we did for the seven records above and have not yet done for the rest.

Why compounded tirzepatide costs more than compounded semaglutide

Across providers, compounded tirzepatide runs roughly $50 to $100 a month above compounded semaglutide. The usual explanation is supply: the active ingredient is harder to source, fewer pharmacies stock it, and the bulk substance costs more.

If price is the only constraint and you have no clinical reason to prefer tirzepatide, compounded semaglutide is cheaper. If you want tirzepatide specifically, the premium is structural rather than negotiable, and the question becomes which compromise you accept — a prepaid year, a dose ladder, or a higher flat rate.

What has changed in 2026

The FDA proposed on 30 April 2026 to exclude semaglutide, tirzepatide and liraglutide from the 503B bulks list. The comment period closed 30 July 2026. Section 503A patient-specific compounding is a separate pathway not addressed by that proposal.

A 2026 study found that tirzepatide compounded with vitamin B12 can chemically bond, forming a molecule not present in the approved drug. Several compounders add B12 as a differentiator. Detail on our additives page.

Brand pricing fell. LillyDirect at $299 to $449 has narrowed the gap that made compounded the only realistic route for uninsured patients two years ago.
